Use and Disclosure of Protected Health Information
PHI includes information in your medical record that identifies you and relates to your physical or mental health, treatment, payment history, diagnoses, care plans, session notes, prescriptions, and insurance details.

Authorization for Other Uses
Any use or disclosure of PHI outside standard treatment, payment, and healthcare operations requires your written authorization, which you may revoke at any time in writing.

Staff are trained in HIPAA compliance and bound by confidentiality. Secure systems are used for electronic records, telehealth sessions, and insurance processing. Business Associate Agreements are in place with third-party vendors handling patient data.
